Are you a Licensed Social Worker who enjoys working with Seniors?  We have a newly vacant opportunity for a caring and compassionate Social Worker (BSW or MSW) to work with residents and their families in a Long Term Care setting.  This positon helps residents navigate through their stay from admission thru discharge at our Senior Living Community in Cadillac.  You will work closely with all members of our amazing team including Admissions, Nursing, Nutritional Services, and Activites to ensure our residents receive the highest leve of quality care and services.

Duties And Responsibilities

Job Summary

 

Assists residents to maintain highest level of psychosocial functioning and well-being. Coordinates admissions for residents. Serves as a key member of the resident care team.

 

Duties and Responsibilities

  1. Advocate on behalf of residents to staff, family and organization.
  2. Identify the social, emotional, and economic needs of residents through a continuous evaluation process.
  3. Participate in the care plan development and implementation of each resident. Provide intake processing as needed.
  4. Document plans, goals, and observations of each resident in electronic records.
  5. Work with referral sources to identify potential candidates for the center. Coordinate move-ins and required documentation.
  6. Plan and coordinate admissions. Prepare and explain patient contract, programs, and policies to responsible party during admission. Complete all paperwork with the responsible party during the admission process.
  7. Coordinate client's room assignment and arrival with nursing and other departments. Engage with resident/responsible party upon admission. Record in the appropriate database as required.
  8. Coordinate residents' discharge. Complete discharge instructions and refer equipment and ongoing services as needed.
  9. Participate in monthly Quality Assurance and Performance Improvement (QAPI) meetings and initiatives.
  10. Compile necessary reports and statistics and submit to supervisor.
  11. Provide oversight and supervision to all aspects of the social services department, including but not limited to, supervision of assigned staff. 

Qualifications

Job Qualifications

 

Education, Training, and Licensure/Certification

  • Bachelor's degree in Social Work or related field.
  • Licensed as a Social Worker in the State of Michigan preferred.

 

Experience

  • Two years' experience in social work.
  • Experience with older adults and/or persons with disabilities preferred.

 

Knowledge, Skills and Abilities

  • Basic counseling, coaching, and group counseling skills.
  • Ability to display compassion and sensitivity with vulnerable adults who may become angry, hostile or combative. Ability to maintain a helping role and intervene with resident care when appropriate.
  • Ability to communicate effectively with individuals with varying levels of cognitive and physical impairments.
  • Ability and willingness to work constructively with individuals from diverse cultural backgrounds.
  • Ability to apply principles of rational systems to solve practical problems and deal with a variety of concrete variables.
  • Knowledge of and ability to employ social brokering, advocating and mobilizing individuals and groups.
  • Basic knowledge of state and local human service systems and resources, particularly as they apply to geriatrics.
  • Ability to work independently and cooperatively. Ability to apply practical mathematical applications.
  • Excellent oral and written communications skills with the ability to read, write and speak the fluent English.
  • Ability to prepare business letters, expositions, summaries, and reports, using prescribed format and conforming to all rules of punctuation, grammar, diction and style.
  • Knowledge of various computer software applications.

 

Additional Work Requirements

  • Weekend on-call required.

  • Ability to move freely about campus.
  • Ability to communicate orally and in writing.
  • Finger and hand dexterity.
  • Hand-eye coordination.
